Please fill out the medication questionnaire (click the box below). Once approved you will receive an email with instructions on how to get your medications. This will include an online 5 - 10 minute Health Consultation (Cost $49) with a Healthcare Provider. The Health Consultation is required by law before any medication can be dispensed, for ALL PATIENTS.
